Accepted Insurance Plans

Although we accept most major insurance plans, it is imperative that you call your insurance and verify that we are a participating provider. To do so, you will probably need to give them our Tax I.D. number: 742905204.  If we are deemed "out of network", you may still visit us, but most insurance companies have an "out of network deductible" that must be met.
